note to  inform conference planners of your intent to come 
to the conference. Print this also if you still have time to 
send registration before arrival.  Please be certain of 
your attendance if you use this to notify for participation 
in meals and you are paying at the door. 
Without meal participation there is no charge, but we 
still want to hear from you if you decide to attend at 
the 11th hour. The cost of  lunch and snacks $20  
($10 for low-income / student; full scholarship available 
for medical students and residents
